Assessment
This is a 54-year-old male with uncontrolled type 2 diabetes mellitus, diagnosed 6 years ago, presenting for initial endocrinology consultation due to worsening glycemic control. Patient reports classic hyperglycemic symptoms including polydipsia, polyuria (especially nocturia), fatigue, and intermittent blurred vision over the past 4-5 months. Current A1c is 9.4%, increased from approximately 8% one year ago. Home glucose monitoring reveals fasting blood sugars 170-220 mg/dL and postprandial readings exceeding 250 mg/dL. Patient is currently on metformin and glipizide but reports suboptimal adherence, missing several doses weekly due to work schedule and travel.
Patient has symptoms concerning for diabetic peripheral neuropathy, including bilateral lower extremity paresthesias, tingling, occasional burning pain, and decreased sensation. Physical exam reveals diminished monofilament sensation in at least one area of the feet. Patient has not had ophthalmology evaluation in 3 years and reports intermittent blurry vision, raising concern for diabetic retinopathy. Symptoms of obstructive sleep apnea are present (loud snoring, witnessed apneas, non-restorative sleep, daytime fatigue), which may be contributing to poor glycemic control.
Additional comorbidities include hypertension (blood pressure today 146/88 on lisinopril), hyperlipidemia (on atorvastatin), obesity (BMI approximately 34.1 kg/m² based on height 5'10" and weight 238 lbs), and possible non-alcoholic fatty liver disease (elevated liver enzymes per patient report). Strong family history of type 2 diabetes and cardiovascular disease (father died of MI in early 60s). Former smoker with 20 pack-year history, quit 5 years ago.
Lifestyle factors contributing to poor control include sedentary occupation, minimal exercise (walks 1-2 times weekly), weight gain of 20 pounds over 3 years, and diet high in refined carbohydrates and regular soda. Patient has never worked with diabetes educator or nutritionist. Infrequent self-monitoring of blood glucose due to finger-stick aversion. Patient reports rare hypoglycemic episodes (lowest recorded glucose 62 mg/dL) with appropriate symptom recognition.
Patient is motivated to improve diabetes management, expressing concerns about developing complications similar to his father (vision loss, kidney disease). Goals include weight loss, improved glycemic control, and increased energy. Patient expresses interest in diabetes technology including continuous glucose monitoring.
Plan

Diabetes Management:
- Intensify diabetes regimen given A1c 9.4% and symptomatic hyperglycemia
- Start basal insulin (glargine 10 units subcutaneously at bedtime) in addition to current oral medications to improve fasting glucose
- Continue metformin; reinforce importance of medication adherence
- Will reassess glipizide after insulin initiation given history of hypoglycemia
- Prescribe continuous glucose monitor (CGM) to improve glucose awareness and identify patterns
- Target A1c <7%; individualized approach given patient motivation and absence of severe complications

Diabetes Education and Lifestyle:
- Refer to certified diabetes educator for comprehensive diabetes self-management education
- Refer to registered dietitian for medical nutrition therapy focused on carbohydrate management, portion control, and meal planning for travel
- Recommend elimination of regular soda and juice; switch to water, diet beverages, or unsweetened options
- Encourage gradual increase in physical activity to 150 minutes weekly; suggest walking after meals to improve postprandial glucose
- Weight loss goal: 5-10% body weight reduction over 6 months

Diabetic Complications Screening:
- Order comprehensive metabolic panel, HbA1c, lipid panel, liver function tests, and urine albumin-to-creatinine ratio to assess renal function and screen for diabetic nephropathy
- Refer to ophthalmology for dilated fundoscopic examination to screen for diabetic retinopathy (overdue)
- Diabetic peripheral neuropathy clinically evident; will initiate pharmacologic therapy

Neuropathy Management:
- Start gabapentin 300 mg at bedtime for symptomatic diabetic peripheral neuropathy (tingling, burning); may titrate as needed
- Educate on daily foot inspection, proper footwear, and foot care
- Recommend podiatry evaluation

Cardiovascular Risk Reduction:
- Continue atorvastatin for hyperlipidemia; will check lipid panel
- Blood pressure suboptimal at 146/88; may need to increase lisinopril or add second agent after reviewing home readings
- Consider adding GLP-1 receptor agonist at future visit for additional glycemic control, weight loss, and cardiovascular benefit

Sleep Apnea Evaluation:
- Symptoms suggestive of obstructive sleep apnea (loud snoring, witnessed apneas, non-restorative sleep, daytime fatigue, obesity)
- Refer to sleep medicine for evaluation and possible polysomnography
- Untreated OSA may contribute to insulin resistance and difficult-to-control diabetes

Follow-up:
- Return to endocrinology clinic in 6-8 weeks to review CGM data, assess insulin titration, review laboratory results, and adjust treatment plan
- Encourage patient to contact office with questions about insulin administration or hypoglycemia
- Provide insulin teaching materials and emergency glucagon prescription

Preventive Care:
- Recommend annual influenza vaccination
- Ensure up to date with other age-appropriate immunizations
Patient is motivated and engaged. Discussed importance of medication adherence, lifestyle modification, and regular follow-up to prevent long-term diabetes complications. Patient verbalized understanding and agreement with plan.
